Do you take my insurance?

Physio Chicago is currently in network with Blue Cross Blue Shield, United Health Care, and Medicare. We also take most other major commercial insurances out of network. Prior to your first visit we will contact your insurance and verify your eligibility and benefits. We will also let you know if you are responsible for any costs based on your deductible, out of pocket, co-pay, or co-insurance.

Do I need a script from my doctor?

Illinois is a direct access state allowing you to see a physical therapist without requiring a script from a physician. Some insurances, though, do require one such as Medicare. When you call and schedule your evaluation we can let you know if you need one.

If you don’t take my insurance, how can I pay?

We take out of pocket payments per session with the initial evaluation costing $150 and follow up visits costing $100.

How long are sessions?

Physical therapy appointments typically are around 1 hour.

What do I need to wear?

Please wear comfortable clothing that you are able to move around in.

Do you update my doctor on my progress?

Yes, we send frequent progress notes to your physician’s office to keep them updated on how you are progressing in therapy.

Do you have parking?

We have 2 designated parking spots for our patients at our Old Irving Park clinic. There is also free parking on Lowell Ave and ample metered parking on Irving Park Rd.

At our Bucktown clinic, there is ample street parking available on Damen Ave, Cortland St, and Moffat St.
